Specialist Social Worker Supported Living

Out Of Hours Senior Care Organiser

You will work with people who are known to adult social care, and who have been referred into the out of hours service. This may be when a change in circumstances has resulted in risks to a person’s independence or safety. You will support people through such a short-term period of crisis to enable them to remain living independently or to minimise risks.
You will work collaboratively with the Hospital, Active Recovery, other multi-disciplinary Out of Hours Teams, locality teams and Safeguarding Teams to provide flexible and responsive support to people to live safe and independent lives.

The purpose of the team is to provide support to families to help children stay safely at home wherever possible. The Team aims to support families by providing the right help at the right time, which could involve short breaks, positive activities, parenting advice, behaviour support.

We are looking for an operational manager to lead a team to deliver the key strategic aims of the organisation and fundamental changes in policy.
The aim of the team is to provide support to those who need it most whilst supporting people to remain as independent as possible.
The post holder will have significant professional leadership skills to develop a service which has a commitment to the principles of personalisation and solution focused interventions. This will involve creating a team culture which will encourage the development of an informed vision and shared learning.

The successful applicant will work within various care settings such as residential homes, day centres, extra care and supported living settings to observe people with dementia over the course of a typical day and record, using set codes, their actions, behaviours and wellbeing.
They will use a variety of tools that will enable them to write comprehensive reports that include recommendations and identify key objectives to achieving “excellence” in Dementia Care across the city.
