We provide consistent and intensive case management during the initial stages of a service being set up or where the professional view is that support can be reduced over a period of time e.g. over the first years of care, which include young people transitioning from children into adults services.
The successful candidate will be based in the High Needs team working directly with the specialist mental health and learning disability commissioner to plan, prepare and deliver bespoke services for people with complex needs, living in supported living environment.
